The Oregon Vital Statistics is responsible for issuing certified copies of vital records, including birth, death, marriage and divorce certificates for events that occurred in Oregon. The office has birth and death records dating from July, 1903; marriage records from 1906 and divorce certificates (not decrees) from 1925.
Effective December 17, 2012, the Center for Health Statistics has a new number for ordering vital records.
- To order a vital record over the phone, you will need to call the new
number 888-896-4988. Phones are answered by VitalChek operators
24 hours a day, 7 days a week except for major holidays.
- The cost for one record ordered over the phone is now $38.45.
- For information on how to order records or make changes to records, call the
previous number (971-673-1190).
